Plan 020 (Harmony) and Plan 032 (Hope-Drug Savings)
(Schizophrenia, Schizoaffective disorder, Bipolar, and Major depression for individuals under 65 years old)
This program provides a specialized support system for individuals who are diagnosed with Schizophrenia, Schizoaffective disorder, Bipolar, Major Depression, and Paranoid disorder.
The Brand New Day specialized program allows for the members to have direct access to a coordinated team of doctors and case managers to support the healthcare needs of our members. The team includes a personal medical doctor, a psychiatrist, a behavioral health trained life coach, a day treatment program, and based upon need, a nurse. Each of these healthcare professionals work in a coordinated way to care for our members. The assigned case managers work with each member to help develop a plan to address any behavioral or health issues and recommended preventative health services.
The program includes access to Brand New Day specialized care management programs, such as:
- Addiction programs
- Smoking cessation programs
- Special program for individuals with pulmonary (lung) issues
- Special program for individuals with diabetes
- Special program for individuals with CHF
- Home nursing program
- Housing support program
- 24 hour nurse advice line
- Assistance with coordinating Medicare and Medi-Cal benefits (for those members with both Medicare and Medi-Cal benefits)
Brand New Day has two benefit plans to choose from. One plan (Plan 032-Hope) has reduced pharmacy costs (HMO SNP Drug Savings Plan), and one plan (Plan 020-Harmony) has additional benefits and higher pharmacy costs (Extra Benefit Plan). Members who have both Medicare and Medi-Cal coverage often choose the extra benefit plan since the Medi-Cal program pays for many of the non-covered benefits costs.
