Register hereMolina Healthcare provides several Disease Management and Health Management programs designed to assist members and their families with better understanding how to self-manage their chronic health conditions, adopt healthy lifestyle behaviors and follow their providers treatment plan. Members receive telephonic outreach from clinical case managers or health educators who complete a condition-specific assessment, develop an individualized care plan and provide health education in accordance with clinical guidelines. Health education materials are sent to members to provide additional support and information. The programs include:
For child and adult members age 2 and older who have a confirmed diagnosis of asthma. Members and parents/guardians will learn about symptom identification and control, trigger avoidance and the importance of following the asthma action plan outlined by their provider. For adult members age 18 and older who have a confirmed diagnosis of diabetes (non-gestational and/or non-steroid-induced). Members will learn about limiting carbohydrates in their diet, monitoring their own blood sugar, knowing their HgA1c level, the importance of daily activity and following the diabetes management plan outlined by their provider. For adult members age 18 and older who have a confirmed diagnosis of hypertension or heart failure. Members will learn about heart-healthy eating, monitoring their weight and blood pressure, the importance of daily activity and taking their medications as directed by their provider. For adult members age 18 and older who either have a confirmed diagnosis of depression or may be struggling with symptoms of depression. The program is designed to provide better overall quality of life and clinical outcomes by administering a standardized depression assessment tool for early identification of symptoms and by providing education, guidance and support, with development of goals focused on self-advocacy for the member and their support systems. For adult members age 35 and older who have a confirmed diagnosis of COPD. Members will learn about breathing exercises, planning and pacing daily activities, oxygen-related safety measures and the importance of taking their medications as directed by their provider. Molina collaborates with the California Smoker’s Helpline to offer telephonic smoking and vaping cessation counseling. It is available to all members including teens, adults, tobacco chewers and pregnant smokers with doctor’s approval. Counseling services are available in six languages. Molina covers Nicotine Replacement Therapy. PA is required for some NRT’s. You can refer members electronically or to call 1-800-NO-BUTTS or 1-844-8-NO-VAPE.
Molina Healthcare offers a Weight Management Program to support the weight management and health nutrition needs of your adult patients, age 18 and older (excluding those members scheduled for bariatric surgery). A specially trained Health Educator works closely with your patients to develop a weight management plan of care that is individualized to meet your patients needs and assist them in achieving their weight loss goals. MHC has teamed up with Teladoc Health, Inc. to bring our members the Diabetes Prevention Program. Teladoc Health uses a mobile app and the program is recognized by the Centers for Disease Control and Prevention (CDC). It is a lifestyle change program, developed to prevention type 2 diabetes and is designed for members who have prediabetes or are at risk for type 2 diabetes. This program is not for members who already have diabetes. Trained coaches lead the program to focus on healthy eating, stress reduction, and physical activity to create long-term changes and lasting results. Molina Healthcare offers a Nutrition Consultation Program to support the nutritional and educational needs of your patients. A Registered Dietitian will work closely with your patients and their guardians/caregivers to assess their nutritional and educational needs and develop an appropriate nutrition plan of care that is individualized to meet the members needs and provides members with more of the tools and resources they need to enhance their ability to self-manage their health condition(s).
