To receive your reward, complete your health exams as soon as possible.
Once you have completed your screening, complete the attestation on the Member Portal.
Have you registered to use the portal?
If you haven’t, please follow the steps below to register for the portal. If you have already registered, you may need to create a new password.
If you have never registered, here’s how:
- Go to MyMolina.com
- Click on ‘Create an Account’
- Have your valid email address, enrollment state, member ID, and date of birth ready to go
- Create a username and password
- Use your valid email address so you can get a code to verify that it is you. You can also add your mobile phone as another option for verification
- Enter that code into your registration screen and you’re logged in
Other things to know about the portal
Your registration information will work on the web version from your computer, iPad or phone browser. You can download the app version as well. Visit your Android or Apple app store to find the My Molina app or use your phone camera to scan this QR code for easy access to the portal.
