Molina Medicare (HMO D-SNP)

Molina Medicare (HMO D-SNP) is a Medicare Advantage Prescription Drug Special Needs HMO Plan, for people with both Medicare and Medicaid. The plan offers members all the benefits of Original Medicare and much more.

You are eligible to join the Molina Medicare (HMO D-SNP) plan if:

  • You reside in the Molina Medicare (HMO D-SNP) geographic service area
  • – and – you are entitled to Medicare Part A
  • – and – you are enrolled in Medicare Part B; and you or Medicaid continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party
  • – and – you are entitled to Medicaid
  • – and – you are in a qualified dual eligible coverage category (FBDE, SLMB+, QMB+)

 

Molina Medicare Select (HMO D-SNP)

Molina Medicare Select (HMO D-SNP) is a Medicare Advantage Prescription Drug Special Needs HMO Plan, for people with both Medicare and Medicaid. The plan offers members all the benefits of Original Medicare and much more.

You are eligible to join the Molina Medicare Select (HMO D-SNP) plan if:

  • You reside in the Molina Medicare Select (HMO D-SNP) geographic service area
  • – and – you are entitled to Medicare Part A
  • – and – you are enrolled in Medicare Part B; and you or Medicaid continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party
  • – and – you are entitled to Medicaid
  • – and – you are in a qualified dual eligible coverage category (SLMB, QMB, QDWI, QI)

Molina Medicare Choice Care (HMO) is a Medicare Advantage Prescription Drug Special Needs HMO Plan, for people with both Medicare and Medicaid. The plan offers members all the benefits of Original Medicare and much more.

You are eligible to join the Molina Medicare Choice Care (HMO) plan if:

  • You reside in the Molina Medicare Choice Care (HMO) geographic service area
  • – and – you are entitled to Medicare Part A
  • – and – you are enrolled in Medicare Part B; and you or Medicaid continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party
