Care Management
SWH of NY provides members with a Care Management team and a Care Manager who is a health care professional – usually a nurse or social worker. Care Managers work with members, their doctors, and their Care Management team to decide the services they need and develop a care plan.
Care Managers work with members to create a comprehensive Person-Centered Service Plan (PCSP) appropriate for their care and service needs. Care Management helps members get the covered services identified in their PCSP as well as medical, social, educational, financial and other services that support their PCSP. Care Managers make home visits at least twice a year to review members PCSP and service needs. Members also receive at least one monthly telephone call from their Care Management team.
Care Managers are available 24/7
Members can call to speak with a Care Manager 24/7. We will respond to calls received on weekends, holidays and after hours on the same day they are received by the on-call Care Manager.
Flexible person-centered care planning
We understand needs and services can change. We provide flexibility in developing a PCSP that meets members individual needs. Our benefits include a wide array of long-term support services and home- and community-based services that help maintain independence as members stay at home and in their community. Care Managers work with members to create a comprehensive Person-Centered Service Plan (PCSP) appropriate for their care and service needs. Members will receive their approved PCSP within 15 days of being enrolled in SWH of NY and again during their reassessment or annual assessment.
