Senior Whole Health Medicare Choice Care (HMO) is a Medicare Advantage Prescription Drug HMO Plan for people with Medicare. The plan offers members all the benefits of Original Medicare and much more.
You are eligible to join the Senior Whole Health Medicare Choice Care (HMO) plan if:
- You reside in the Senior Whole Health Medicare Choice Care (HMO) geographic service area
- – and – you are entitled to Medicare Part A
- – and – you are enrolled in Medicare Part B; and continue to pay your Medicare Part B premium

Senior Whole Health (HMO D-SNP)
Senior Whole Health (HMO D-SNP) is a Medicare Advantage Special Needs plan for seniors aged 65 and over that combines your Medicare, MassHealth Standard (Medicaid) and Medicare Part D benefits in one convenient plan.
You are eligible to join the Senior Whole Health (HMO D-SNP) plan if:
- You reside in the geographic service area
- – and – you must have both Medicare Part A and Medicare Part B
- – and – you are eligible for MassHealth Standard (Medicaid)

Senior Whole Health NHC (HMO D-SNP)
Senior Whole Health NHC (HMO D-SNP) is a Medicare Advantage plan for seniors aged 65 and over who are eligible for both Medicare and Medicaid benefits. Individuals requiring nursing home level of care receive home and community-based support services that allow them to remain comfortably in their own homes.
You are eligible to join the Senior Whole Health NHC (HMO D-SNP) plan if:
- You reside in the geographic service area
- – and – you must have both Medicare Part A and Medicare Part B
- – and – you are eligible for MassHealth Standard (Medicaid)

Senior Whole Health Senior Care Options (SCO)
Senior Whole Health SCO plan offers Medicaid, Medicare and Part D benefits to seniors with MassHealth.
You may qualify if:
- You are 65 years or older
- You have MassHealth (Medicaid)
- You live in our service area

In certain situations, you will be denied enrollment in Senior Whole Health
Massachusetts seniors will be denied enrollment to Senior Whole Health Medicare Choice Care (HMO), Senior Whole Health Senior Care Options (SCO) plan, Senior Whole Health (HMO D-SNP) or Senior Whole Health NHC (HMO D-SNP) if:
- – You do not meet the eligibility criteria listed above
- – You are subject to a six (6)-month deductible period (spend-down)
- – You are a resident of an intermediate care facility for the intellectually disabled
- – You are a patient in a chronic disease or rehabilitation hospital
