Children’s Home and Community-Based Services
Children’s Home and Community-Based Services are strength-based services for children and youth up to age 21 who have serious emotional difficulties, developmental difficulties and/or are medically fragile. The services are intended to support children and their families or caregivers where they are most comfortable – at home or in the community. Eligibility for these services is determined by an assessment conducted either by a Children’s Health Home or New York State’s Independent entity, C-YES.

- Bereavement Services
- Non-Medical Transportation
