Senior Whole Health Medicaid Managed Long Term Care Program
Senior Whole Health of New York (SWH of NY) provides long-term care benefits and services for adults with Medicaid in Bronx, Kings, New York, Queens and Westchester counties. We help you live safely and independently at home for as long as possible. All members get a Personal Care Manager who works with you, your doctors and your family to ensure your care plan meets your individual needs and goals. You get the added peace of mind that comes with knowing you have 24-hour access to your care management team.
