What you should know
- Only primary Medicaid Molina members can get Healthy Rewards
- Annual Screenings must be completed between January 1st and December 31st of the calendar year. Requested information must be submitted no later than 20 days after December 31st of the calendar year
- All services are verified through claims data before payment
- Benefits are subject to change
