Healthmap Solutions (Healthmap) is the approved vendor for our Kidney Health Management (KHM) program. Healthmap improves care and outcomes for eligible members with chronic kidney disease (CKD) and end-stage renal disease (ESRD).
The Healthmap program focuses on early detection of kidney disease, proper care, and treatment to improve health outcomes. This helps members live a better and longer life. The program also helps the relationship between you and your provider. This is because Healthmap gives your provider’s office information to give you the right care, at the right time. You may also receive support for your health care needs from Molina Healthcare (Molina) employees while in this program.
Your privacy is important to us. Molina and Healthmap have a Business Associate Agreement to make sure your information is protected under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). This means that your health information is safe and only shared with a Molina partner (Healthmap) who understands the sensitivity of this information.
Healthmap may contact your provider’s office to receive or share Protected Health Information (PHI) about you. This is needed to give you Healthmap care coordination services. These communications are approved by Molina and allowed without member approval under HIPAA and as outlined in Molina’s Notice of Privacy Practices.
Your Healthmap Care Navigator will work with you between doctor visits to answer questions, provide education, help set appointments, manage your medications, and more.
