SCDHHS Online Portal for Address Change Requests
Are you a Healthy Connections Medicaid member? Have you moved? SCDHHS has a web portal to make it easier to update your address. This will help you receive any information about any changes in your Medicaid.
You can change your address online at https://apply.scdhhs.gov. It is important to keep your contact information up to date. If we do not have current information, you could miss updates about your Medicaid coverage. To update your home or mailing address, you will need your Medicaid card or Social Security number, date of birth, phone number and new address.
You can also update your information by phone at (888) 549-0820 Monday through Friday from 8 a.m. to 6 p.m., or by visiting your local eligibility office. Learn more here.
