Q. How do I change my primary care provider (PCP)?
A.There are two ways you can change your PCP.
- Go to www.mymolina.com and follow the instructions. If you have not registered for MyMolina yet, you can do it online today.
- You can call Member Services at (888) 999-2404 or TTY 7-1-1.
Q. What drugs are covered? Are my drugs covered?
A. For members in Wisconsin, prescription drugs are covered through Fee-for-Service Medicaid (the state). Please contact the state using the phone number on the back of your Forward Health ID card for questions regarding prescription drugs.
Q. How do I find out if a health care service or benefit is covered?
A. Go to “What’s Covered” to find out about your benefits and services. You can also call Member Services at (888) 999-2404 or TTY 7-1-1.
Q. How can I get Molina Healthcare material translated in my preferred language?
A. If you need your health plan information in a different language or format,call Member Services at (888) 999-2404 or TTY 7-1-1.
Q. What kind of transportation do I get? I need help with transportation.
A. Non-emergency medical transportation to medical facilities and pharmacies is covered through Fee-For-Service Medicaid. To arrange for non-emergency medical transportation, call (866) 907-1493. Please try to call 48 hours in advance. You can also go to “What’s Covered” to find out what transportation services are covered.
