State Fair Hearing
If you do not agree with our appeal decision, you have the right to request a State Fair Hearing. A State Fair Hearing is an appeal that goes to the Nebraska Department of Health and Human Services (DHHS) instead of Molina. For a State Fair Hearing, Nebraska DHHS will make the final decision.
You have the right to request a State Fair Hearing within 120 days from the day you receive our appeal decision.
You must submit your State Fair Hearing in writing to:

Be sure to include the following:
- Member’s first and last name.
- Molina Healthcare ID number. This is on the front of the Member ID Card.
- Member’s address and telephone number.
- Explain the problem.
