Care Connections
Care Connections contributes to the Molina/Passport mission by expanding access to quality care by meeting Enrollees where they are, including home visits, mobile clinics, pop-up clinics, and virtual visits. Care Connections nurse practitioners offer wellness and preventive care services and “boots on the ground” to determine if social determinants play a role in a member's health challenges.
